Durham Medical Centre has established a connection to HRM® (Health Report Manager), the province’s electronic report delivery system. We can now send copies of your medical record and diagnostic imaging reports to your primary care provider (family physician or nurse practitioner) so they can review the information and file it in your chart in their electronic medical record system. This sharing of information facilitates more informed decision making and faster follow up by your primary care provider.
